Weight Loss Program Referral Rewards — Terms & Conditions

Rewards are issued after verification of a qualifying intro-offer redemption via your unique link; one reward per referred person. Rewards are issued as an En Vogue gift certificate/gift card (digital or physical) with no cash value, non-transferable, and usable for any service or toward an existing membership autopay. Not combinable with some promotions. Clinical eligibility and provider discretion apply. Individual results vary. Terms subject to change.

Full Terms

1) Definitions

• Referrer: An individual sharing their unique referral link.
• Referred person: Someone new to the En Vogue IV Therapy & Wellness Weight-Loss Program.
• Qualifying intro offer: The new-patient introductory offer for the Weight-Loss Program in effect at the time of redemption.
• Successful referral: The referred person (a) uses the referrer’s unique link, (b) completes required registration and clinical evaluation, and (c) redeems and is charged for the qualifying intro offer (no cancellation, refund, or chargeback).

2) Eligibility

• Referred persons must be new to the Weight-Loss Program. Self-referrals and referrals of current participants are not eligible.
• Family, friends, and coworkers are welcome.
• The first link attributed at sign-up earns the reward; referral credit is not retroactive.

3) Reward amount

• $100 (gift certificate/gift card) for your first successful referral; $75 for each additional successful referral.
• One reward per referred person. Unless otherwise posted, there is no overall cap on different people you may refer.

4) Verification & issuance

• Rewards are issued after verification; allow reasonable processing time.
• If the qualifying redemption is canceled, refunded, or charged back, the reward may be voided or reversed.
• We may request information needed to confirm eligibility and attribution.

5) Format & nature of reward

• Issued as an En Vogue gift certificate/gift card (collectively, “Reward Credit”), which may be digital (code) or physical.
• No cash value, not reloadable, no cash back, and does not accrue interest. Not FDIC-insured.
• Lost, stolen, or misused Reward Credit may not be replaced.

6) Using your Reward Credit

• May be used for any service at En Vogue IV Therapy & Wellness or applied to an existing membership autopay.
• If applied to autopay, credit is applied to the next scheduled charge after issuance and cannot be used to pay past-due balances.
• Not combinable with some discounts, promos, or packages unless expressly stated.
• Standard scheduling policies, service-specific terms, and clinical eligibility apply.

7) Anti-fraud & fair use

• We may withhold, void, or reverse rewards for suspected fraud, self-referral, spam, paid lead-gen, or rule violations.
• We may audit and adjust referral attributions to ensure accuracy.

8) Privacy & PHI

• Do not include personal health information when sharing your link.
• We will not disclose a referred person’s medical details to referrers; you may receive reward-status updates only after verification.

9) Advertising & disclosure (FTC)

• If you publicly post or promote your link, clearly disclose that you may receive an incentive (e.g., “I may earn an En Vogue gift certificate for referrals”).
• Endorsements must be truthful and typical; avoid claims about outcomes.

10) Changes; governing law

• En Vogue may modify, suspend, or terminate this program or these terms at any time.
• Governed by the laws of the Commonwealth of Pennsylvania; venue/jurisdiction in Pennsylvania courts.

11) Medical & results disclaimers

• Participation in the Weight-Loss Program requires clinical eligibility and provider discretion; not all individuals qualify.
• Individual results vary. Information provided is not medical advice.
